Regular membership notes

• *There is a one-time enrollment fee of $200 (individual or family).

• Membership is an annual contract. You may cancel at any time with a 30 day written notice.

• Re-enrollment fee of $250 will be applicable if members cancel then re-enroll at a later date.

• There are no restrictions on frequency, type or duration of office visits per month.

• Costs of medications, immunizations and some procedures not included.

• Membership pricing is subject to change in the future.

• Children NOT part of a family membership will be charged $75 a month
